OMB Approves OFCCP’s Revised Disability Self-ID Form CC-305

What's New

The White House Office of Management and Budget (OMB) has approved OFCCP’s request to continue using its prescriptive disability self-identification Form CC-305 for another three years with only minor changes. Covered federal contractors must begin using the revised CC-305 July 25, 2023.

What It Means

The new Form CC-305 contains the following minor revisions to the outgoing form:

  • It lists the examples of disabilities in alphabetical order;
  • It adds examples and updates the list of disabilities to reflect the current preferred language for specific disabilities;
  • It adds language in bold font clarifying that the listed disabilities are not exhaustive; and
  • It replaces the term “substance use disorder” with the term “alcohol or other substance use disorder.”

Contractors are not allowed to modify the form in any way except by editing or deleting the "For Employer Use Only" section. When incorporating the new Form CC-305 into their HTMS or IT systems, contractors or their vendors must confirm that the form has the proper OMB control number and expiration date.
